Certain safety-sensitive occupations may require an examination by an employer-designated provider or occupational medicine specialist.

Every Request Is Reviewed Individually
A requested form cannot be completed solely because it has been submitted. All statements must accurately reflect the patient’s medical history, current condition, functional limitations, and available clinical evidence.
We do not provide unsupported, inaccurate, misleading, or backdated documentation. The employer, insurer, school, government agency, or other receiving organization makes the final decision regarding eligibility, benefits, accommodations, or approval.

Important Limitations and Ethical Standards

Dr. Hu will only complete forms, letters, certifications, or medical statements that are supported by the available medical evidence, within his professional scope, and consistent with his independent clinical judgment.
Documentation Must Be Medically Supportable
Every diagnosis, limitation, accommodation, recommendation, or certification must be supported by your medical history, evaluation findings, available records, and other relevant clinical information. A requested statement cannot be added solely because a patient or third party asks for it.
Additional Evaluation May Be Required
Depending on the request, an appointment, physical examination, diagnostic testing, specialist evaluation, or additional medical records may be needed before documentation can be completed. In some situations, Dr. Hu may determine that there is not enough information to provide the requested opinion.
Approval Cannot Be Guaranteed
Completing medical documentation does not guarantee approval by an employer, school, insurance company, government agency, court, attorney, benefits administrator, or other reviewing organization. The receiving party makes the final decision according to its own requirements and eligibility criteria.
Some Requests May Require Another Clinician
Certain legal, forensic, psychiatric, occupational, disability, functional-capacity, or specialty-specific requests may require an established treating specialist, an independent medical examination, or a clinician with particular qualifications. When appropriate, we will explain the limitation and recommend a more suitable next step.
Information Must Be Accurate
Documentation cannot contain information that is inaccurate, misleading, unsupported, intentionally incomplete, or backdated. Forms will only be signed after all required information has been reviewed and the physician determines that the statements can be medically supported.
Privacy and Authorization Are Required
Your medical information will be handled carefully and confidentially. Written authorization may be required before records or completed documentation can be released to an employer, attorney, insurer, government agency, family member, or another third party.

Medical documentation requests vary in length and complexity. Our pricing is based on the number of completed documentation pages and whether a physician consultation is needed.
We will review your request to determine what information, records, and physician involvement may be required.
Minimum Documentation Service Fee
$150
A minimum fee of $150 applies to every medical documentation request, whether or not a physician consultation is required.
Requests involving additional pages, consultation time, extensive record review, or greater complexity may have a higher total cost.
Documentation Preparation
$50 per completed page
Medical documentation is billed at $50 for each completed page prepared by the physician.
The minimum total charge for documentation services is $150, even when the requested documentation is fewer than three pages.
Physician Consultation, When Needed
$150 for up to 30 minutes
A physician consultation may be required when additional evaluation, medical history, examination findings, clinical clarification, or updated information is needed before documentation can be completed.
Additional consultation time is billed at:
$50 for each additional 10 minutes
How the $150 Minimum Works
The minimum charge for any documentation request is $150. When a physician consultation is required, consultation fees are charged in addition to applicable documentation preparation fees.
For documentation requests that do not require a consultation, this minimum generally covers up to three completed pages. Documentation exceeding three pages is billed at the standard rate of $50 per page.
When a physician consultation is required, consultation and documentation fees are calculated according to the time and number of pages involved.
